Intraoperative neuromonitoring

Intraoperative neuromonitoring refers to monitoring of important functions of the nervous system during neurosurgical interventions. It is aimed at timely recognition of nerve structures impaired by intervention as well as localization of functionally important structures before the intervention. Depending on the structure and kind of intervention to be monitored, various procedures, partly simultaneous, are applied. Because of the intraoperative monitoring the surgeon obtains continuous feedback about the performance of the operated region. He can adjust his surgical strategy to the findings, so that maximum protection of important brain structures is ensured during the intervention.

Procedure and fields of application

In our clinic almost all current intraoperative monitoring procedures are applied for appropriate indications and taken care of by specially trained medical engineering and medical staff.

The following table provides an overview of the procedures carried out particularly often in our clinic:

Types of surgery

Type of monitoring

Peripheral nerve surgery

Nerve stimulation + electromyography (EMG)

Cerebellopontine angle tumours (Vestibular schwannoma)

Nerve stimulation + EMG
Acoustic evoked potentials (AEP)

Extracranial interventions on brain supplying vessels (carotid surgery, see image below)

Electroencephalogram (EEG)
Transcranial Doppler sonography (TCD)
Somatosensory evoked potentials (SSEP)

Aneurysm surgery

Microvascular Doppler sonography

Interventions in the vicinity of the central region

Cortical electrostimulation
Somatosensory evoked potentials

Interventions close to the spinal cord

Somatosensory evoked potentials
