Case description
The 63-year-old female patient had complained of suffering headaches over an extended period as well as increasing attention, concentration and memory disorders. The MR imaging shows a large cavum veli interpositi which is partially obstructing both lateral ventricles and displacing the fornices.

Postoperatively the patient reported a significant improvement in her headache symptoms as well as an improvement in the cognitive deficiencies. The postoperative MR images show a clear reduction in the size of the cavum veli interpositi and a wide open septostomy.
