Case description

The 48-year-old male patient presented with headaches, gait disorders and mental deterioration.

MR imaging shows an obstructive hydrocephalus with clear enlargement of both lateral ventricles and the third ventricle. The cause of the hydrocephalus is identified as an enhancing tumor in the pineal region. Due to the significant expansion of the ventricles, the brain stem has been displaced caudally. The floor of the third ventricle bulges into the prepontine cistern. The flow-sensitive inversion recovery sequence shows no flow in the aqueductal area.

Result

The patient's symptoms resolved quickly after surgery. The MR images taken four months postoperatively show a clear reduction in ventricle size. The brain stem position has normalized The flow-sensitive inversion recovery sequence shows a strong flow phenomenon in the ventriculostomy area. The histologic diagnosis showed atypical plexus papilloma. The tumor was subsequently removed microsurgically via an infratentorial supracerebellar approach.
