This 50-year-old female was diagnosed with a small tuberculum sellae meningioma as an incidental finding. Despite normal neurological findings, the patient wished to have the tumor removed because of its proximity to the optic nerves. After being informed about the advantages and disadvantages of the transcranial and endonasal approaches, the patient decided on endonasal tumor resection.

The patient displayed no neurological deficits after surgery. MR imaging one year after the operation showed complete removal of the tumor with a well reconstructed skull base.
